Evelyn Marie Prochazka Goshen , New York Evelyn Marie Prochazka, 89, of Goshen died Saturday, December 8, 2012 at Schervier Pavilion, Warwick , NY . Evelyn was born December 22, 1922 in Middletown , NY , the daughter of Walter and Edna (Neely) Sowash. She was the widow of George Prochazka. A loving mother, she is survived by her five children, Frederick Prochazka and wife Beverly of Goshen,NY, Marie Hurteau and husband Paul of Schenectady,NY, Marlene Lysack and husband Joseph of Warwick, NY, Connie Kosior and husband Kenneth of Florida, NY, and George Prochazka and wife Joan of Grahamsville,, NY, nine grandchildren, Debra Pawliczak and husband John, Fred Prochazka Jr. and wife LoriAnn, Todd Hurteau and wife Darcie, Keith Lysack and wife Justine, Beth Purta and husband Tim, Ken Kosior Jr. and wife Regina, Meghan Herrington and husband Matt, Michele Wilbur and Karen Wilbur; and seven great-grandchildren, Taylor, Alex, Chloe, Cate, Jack, Holly and Leah. Mom, Gram, G.G. - we will all miss you but you will remain in our hearts forever. Our family would like to thank the staff of Schervier Pavilion, Forest Hall, for the outstanding care they gave Mom during her time there. Visitation will be Tuesday, December 11 from 2 to 4 p.m. and 6 to 8 p.m. at the Donovan Funeral Home, Inc, 82 South Church Street , Goshen , NY . The Funeral will be Wednesday, December 12 at 11:00 am at the Warwick Reformed Church, 16 Maple Ave., Warwick, NY . Burial will follow in Florida Cemetery, Florida, NY.
